Try removing all instances that you are not logged into from the accounts settings.

Subscribing to communities from beehaw, lemmy.ml, and a few others stuck on "subscription pending" in c/lemmyworld@lemmy.world

Try unsubbing and subbing again, worked for me when it happened.
