I use passbolt and it does have a couple of different fields and tags that you can use. It is in active development and new features comes in fairly regularly. It depends on browser addon that does the heavy lifting and if you switch browsers (or computer etc.) you will need to do account recovery to gain access to your passwords. “Bad” thing is that you can’t access webgui without addon.
